The Dementia Navigator Service is provided by the Alzheimer’s Society and commissioned by Warwickshire County Council and Warwickshire Public Health. The service covers the county of Warwickshire and is designed to support people with dementia and their carers and families at any stage during the progression of dementia. This tailored support, identified via robust assessment, is provided in four ways:

Information – examples include

  • Factual information on dementia, tailored to specific diagnoses
  • Information on support groups/activities
  • Information on cognitive enhancers
  • Legal, financial and welfare information

Advice – examples include

  • Coping strategies
  • Care options and planning for the future
  • Coping with forgetfulness and day to day living

Signposting and referral – examples include

  • Benefit checks and applications
  • Statutory services, including liaison with the Memory Service, GP etc.

Emotional Support

  • For both the person with dementia and carers to support throughout the ongoing challenges of a progressive disease

Facts and Figures from April 2016 to March 2017

346 new referrals made to the service with 95% of those referrals responded to within one working day of the referral being received

A total of 734 clients of the service

770 visits made; outcomes fully met on 80% of all closed cases

1952 support phone calls made
